(3) If you last entered the USA illegally and without border inspection, please give the date (mo/day/yr.) of the entry and any other information.
(4) If you last entered the USA legally and with inspection, please give the mo/day/yr. of the entry.

(5) What type of visa did you last use to enter the USA? (tourist, student, work, etc.) If you currently have more than one visa, please explain.

(6) If you were issued a white ticket (Form I-94) upon your last arrival, what is the expiration date on it (mo/day/yr.)?

(7) Have you ever been charged or arrested for the commission of any crime? Even if the charges were dismissed, please discuss the charge, year of occurrence, and state in which the occurrence took place.

(8) Have you ever been married to a USA citizen, or had children in the USA? Explain, and give dates and ages.
(9) Have you ever remained in the USA on an expired visa, or after the expiration of your permitted stay? If so, for how long? Explain.
